MRI Safety Committee Meeting minutes 1.11.2019 Who attended: Steve Dashnaw (NIB), Wendy Dominguez (MRI supervisor CUMC), Pallavi Utukuri (ALLEN), Valerie Maccarrone (TARRYTOWN), Sachin Jambawalikar, Prachi Dubey, MD (LAWRENCE), Lyndon Luk, Ben Navot, Diego Jaramillo (CHONY), JT Vaughn (ZI/MBBI), Alessandra Capizzi, Michael Liu, Tracy Duffy (MR Manager CUMC), Angela Lignelli Absent: Anthony Taylor, Ariella Harel, Marc Brown, Josh Weintraub, KellyAnne McGorty, Richard Maldonado, 1. Cardiac monitors: Admin, not present. Dr. Hecht will follow up on status of the order. 2. Leadership: Introductions made, need clarification of all MRMDs, MRSOs, MRMEs (Tarrytown and Lawrence in particular). Wendi is the new MRSO at CUMC and is getting further training with certification course/test planned for March. Michael Liu currently serves as MRSO and MRME for NIB. (Dr. HECHT will clarify this with administration) 3. Education: a. Need to create as clear process/system for everyone to feel more comfortable with device clearance and address all CU sites. We shall create a workflow through email exchange and by committee. (liz will send draft for feedback from group) b. Currently level 2 training (should expand to housestaff) But in addition, need more general education on devices for techs and nursing, once we create the above, we will create an in service, power point for rad staff and housestaff. Major impediment to tech education is scheduling and documenting/feedback, need online courses, in person QA sessions or chat session. ? Testing. Additional Education for technologists - suggested twice a year, Tracy to reach out to Michael Kennedy and other sites to see how they handle getting logistics of continuing education GME Office and Sheik for house staff education Mary Maher on house staff quality counsel Ask GE if they have material i. Considered additional formal training and certification for lead techs eg. consider permitting attendance to local NY-Kanal course (not specifically discussed, but point for next meeting) c. Lyndon to disseminate Contrast Policies and teaching powerpoints for education and reference but does not include a large safety component. d. Need more information about adding educational material to NYP sites Michael Liu to contact GME and GE, Tracy to reach out to Rad Admin as well about funding and planning, KellyAnne should be contacted. IT support, needed to make this work as well. e. Consider dedicated education taskforce. 4. Patient chart documentation and registry for devices performed at CUMC sites. Perhaps collaborative with NYP sites. a. EPIC- can this be incorporated eg. Device tab- Ben Navot to contact epic team Ray Hutter b. IT, common database, issues is access, Michael Liu, proposed a common website (no HPI) accessible to all. He will send link to group to review what he has already done. Could this be rolled into Epic long term so we have a single repository of patient and device data which would be helpful for clinical, QC and research? c. Also perhaps use PACS folder for how to ID some devices like MRI safe pacemakers for MDs to learn what to look for in term of safe and non safe devices d. Medium term goal of establishing MRI-specific consent paperwork (cross campus) rather than generic consent form. 5. Research: Separate safety concern risk/benefit analysis. Dr. Vaughn liaison for research. Conditional Devices General Policy for handling devices not proven to be conditionally safe (MR Unsafe)? Specific sites have scanned MR Unsafe devices List may help us see what sites are doing
